Four packs, four identifier spaces

Multi-turn visit. Dermatology and report reading. Turns, roles, gaps, and original wording stay in order, with the images visible at that turn. Do not splice patients. Do not write a later lab result into an earlier turn.
De-identified case. One outpatient or inpatient encounter, including TCM inspection and a syndrome label when that is the record type.
Follow-up. Several visits for one pseudonymous patient, in time order. The last diagnosis does not rewrite the first complaint.
Held-out benchmark. Dermatology, reproductive medicine, endocrinology, andrology, and clinical nutrition. For private evaluation such as Medical Gate. Not a training split.

Authorization and de-identification are required before delivery. If a benchmark item still matches a training item, it leaves the benchmark.
